Liposuction In The Beginning

Everyone has heard of liposuction, but some people still associate it with the procedure that first came to the U.S. from France in the 1980s. That procedure was pretty violent. The patient had to be under general anesthesia. Then the surgeon inserted a relatively large cannula through an incision. He or she then moved the cannula back and forth quite aggressively to break lose the unwanted fat cells in the treatment areas. This back and forth movement of the cannula caused two problems: tissue trauma and bleeding. Because of this, early liposuction caused extensive bruising that could take months to fully resolve itself. Also, patients could lose a fair amount of blood. Plus, the wholesale removal of fat could leave a wavy pattern on the patient’s skin in the treatment area.

Today

Liposuction has come a long way since then. Today, Dr. Fedele uses what is called the tumescent technique. After the patient is sedated or receives twilight anesthesia, a saline solution containing lidocaine and epinephrine is injected into the treatment area. The lidocaine acts as a local anesthetic, eliminating the need for general anesthesia. It also firms the fat, making it easier to remove. The epinephrine acts to constrict the blood vessels. This limits blood loss and bruising.

Cannulas are smaller today, requiring tiny incisions and allowing more controlled fat removal.

In certain areas, Dr. Fedele can also add ultrasound energy to the liposuction procedure. This sound wave energy breaks up and partially liquefies the fat, making it even easier to remove in difficult areas.
